If you are playing on an Nvidia graphics card and experiencing FPS spikes which seem to happen regardless of Dolphin settings, you may need to set your "Power Management Mode" in your Nvidia control panel to "Prefer Maximum Performance". Users have anecdotally reported their GPU's running at only a couple hundred MHz without this turned on.
You can find this option by:
Right clicking anywhere on your desktop -> Nvidia Control Panel -> Manage 3D Settings -> Global Settings -> Power Management Mode -> Set to "Prefer Maximum Performance" -> Click Apply
