The game introduced the Wii Balance Board, which the game uses to measure the player's weight and posture as they stand on it during exercise. Matsunaga described the game as a "way to help get families exercising together", and features a wide variety of exercises such as yoga, strength training, aerobics, and balance games. It has proven so successful at creating entertaining exercise, that it has been adopted by health clubs, nursing homes and even used for professional physiotherapy rehabilitation.

Support for the Wii Balance Board was added in {{revision|3.5-1363}}. As there is no way to emulate the peripheral, this game '''requires''' a real Wii Balance Board to function. To use a Balance Board, first set the Balance Board option in the Wiimote properties from "None" to "Real Balance Board". Then press the red "Sync" button in the battery compartment of the balance board, and press "Refresh" if Continuous Scanning is disabled. There is no fast sync (Wiimote 1+2) in the Balance Board, so the sync button must be used each time.
